Attention Biorepository Customers -
As part of our ongoing effort to provide easier processing of required forms, we are now accepting Digital Signatures from established Public Key Infrastructure (PKI) providers on the following Biorepository forms. A public key infrastructure supports the distribution and identification of public encryption keys, enabling users and computers to both securely exchange data over networks such as the Internet and verify the identity of the other party.
If you do not have this capability we are still accepting “hand” signed forms.
The forms below must have a "hand" signature. Digital signature is not available at this time.
We’d like to hear from you if you have other suggested process modifications that might make the transfer process easier.
